UPMC’s strategy emphasizes collaboration between pharmacy, cardiology, and health plan services, using a multi-channel approach to engage and educate patients and members. The program focuses on improving care quality by integrating targeted e-consult programs, comprehensive patient education tools, and innovative care models aimed at optimizing health outcomes.
The initiative includes strategic efforts to enhance therapy management and reduce hospitalizations, which, in turn, helps lower healthcare costs. A key goal is the development of a unified electronic medical record system to streamline care and further improve patient and member management.
